Criteria: Table 15-14 Sensory and Motor Severity (425), Table 15-20 Brachial Plexus Impairment (UEI) (434-435), Table 15-21 Peripheral Nerve Impairment (UEI) (436-444)
Accurately define diagnosis, and assess history, physical examination findings, and clinical studies, as explained in Section 15-1 (385-386) and Section 15.4a (423-425). Grade severity of any sensory and/or motor deficits using Section 15.4b (425) and Table 15-14 (425). Based on objective nerve involvement and severity of sensory / motor deficits identify impairments from Table 15-21 (436-444). Adjust impairment using Functional History and Clinical Studies (e.g., electrodiagnostic studies). If multiple impairments combine at upper extremity level. Impairment values are converted from UEI to WPI using Table 15-11 (420).Figure 15-2 (388) summarizes findings. Entrapments such as carpal tunnel syndrome and cubital tunnel syndrome are rated per Section 15.4f (432-450), and not by Table 15-21.
