Guidance: Section 7.7, Male Reproductive Organs (156-157), describes the process of rating impairment of male reproductive organs. Sexual functions of erection and ejaculation are presented in Section 7.7a, Penis, and impairment values are provided in Table 7-5, Criteria for Rating Permanent Impairment Due to Penile Disease (156). The Guides classifies these levels of impairment in men into classes 1 to 3, depending on their severity, and accounts not only for erectile function but also for ejaculatory, orgasmic, and sensory function. Most often the impairment is class 1 (0% to 10% whole person permanent impairment); this describes the clinical scenario of mild to moderate sexual dysfunction when sexual function is possible, but there are varying degrees of difficulty of erection, ejaculation, or sensation. Class 2 (11% to 19% whole person permanent impairment) implies that sexual function is possible and there is sufficient erection, but ejaculation and sensation are impaired. Class 3 rating of 20% whole person permanent impairment is given if no sexual function is possible. Adjustments are made for age. If the male is below 40 years the impairment is increased by 50% and if age is over 65 years is decreased by 50%. Sexual functioning due to nervous system dysfunction is assessed using Section 13.7d Sexual System Neurologic Impairments (342) and Table 13-21 Criteria for Rating Neurologic Sexual Impairment (342). If sexual functioning is possible but there is difficulty of erection or ejaculation in men or lack of awareness, excitement, or lubrication in either sex the impairment is 1% to 9% whole person permanent impairment. If reflex sexual functioning is possible but there is no awareness the impairment is 10% to 19% whole person permanent impairment. If there is no sexual functioning or awareness present the impairment is 20% whole person permanent impairment.
