Services > Referral Process

Our referral process is easy and we provide you with alternatives to meet your specific needs. Many of our clients refer all ratings to assure accuracy; this reflects best practice. Most reports are provided within 5 working days and rush reviews are available. Your referral options are:

  • Online Referral
    • Use the 'Click Here to Make Referral' button to the left or create a shortcut on your desktop to: http://www.impairment.com/clientlogin
    • Create an account - this step is only needed once - by entering your email address in the right hand box and click 'Submit'.
    • You will be re-directed to a screen that informs you an email has been sent. This validates your email address.
    • You will immediately receive an email from referral@impairment.com. Click on the link at the bottom of the email to access the 'SiteSignup' page and complete your registration.
    • At the 'SiteSignup' page, complete all fields and click “Submit”.
    • After clicking Submit, you will arrive at your Home page. You can refer a new case by clicking on the button “Create New Case”.
    • Once all the case information is complete, click Submit or Submit & Add Attachment if you wish to upload electronic files as well. You will receive an email confirmation.
    • Print the submitted form for your internal purposes and/or to use as a coversheet for records that are faxed to us.
  • Fax, Mail or Email - Complete our referral form and provide us with the records. Records should be faxed to 619-299-7333 or mailed to 8885 Rio San Diego Drive, Suite 310, San Diego, CA 92108. You may also email records as an attachment to referral@impairment.com.

  • Integrated Systems - We are able to work with your IT staff to integrate our online referral system with your software systems. Records can be transferred via FTP.

Please send the impairment rating report, any operative notes, MRI reports and/or electrodiagnostic studies associated with the cases you are referring over.
