and has inspired many innovations. Mindy Brigham, our Vice
President of Operations, keeps her finger on the pulse of your
sentiments, so I've asked her to share what we've heard
recently and how we're responding in this special edition.

dedicated team of co-workers have effective ways to keep in
touch with our many customers.
Our goal is simple: to provide you with the best service
possible, at fees that represent great value.
In order to do that, we have to find out what you like and
don't like, and figure out how we can do even better. This
laser-sharp focus on you is what we call "the voice of the
client". It affects everything we do - from what services we
provide, to our referral process, and to the structure of our
reports. So how do we keep our finger on the pulse of our
clients? We speak with you, visit you, meet you at training
sessions and other events, and receive your feedback via our
website.

that provides exceptional value.
More cost-effective, focused and tactful reports.
With our impairment rating review services (i.e. our expert
critiques of impairment ratings) we have learned that you want
reports that are more persuasive and better accepted by the
physician who performed the original rating (i.e. more
tactful, and less "nit-picky"). Our goal is to provide
constructive feedback, reports that are more focused, and
reviews at a lower price point. You will see this change
immediately with all of our reports. By focusing our efforts
and not including direct content from the reports we review,
we are able to be more efficient with our time, and therefore
provide reviews at a lower fee. Please let us know what you

We have also learned that more of our clients want us to
screen every impairment report they receive - recognizing that
this is a specialty skill and that someone who does not review
ratings on a full time basis cannot possibly determine which
ratings are correct and which are not. Our data reveals that
the majority of impairment ratings are incorrect, and based on
over two thousand reviews this past year we have found that
the original reports averaged 17% whole person permanent
impairment, however if the Guides were applied
correctly they should have averaged 7% whole person permanent
impairment. It is never our intent to attempt to manipulate a
rating higher or lower - our sole focus is defining the
correct rating. According to the NCCI, 20% of workers'
compensation costs are directly related to impairment ratings,
therefore rating accuracy is critically important. You have
asked us to provide more sophisticated data analysis; we are
preparing this. To meet the increasing demands for our
Impairment Rating Integrated Solutions (IRIS) program we are
increasing our staff. (If you are not yet benefiting from

have access to more world-class specialty physicians - we have
also done this.

5th Edition: 101 Webinar Series
The Fundamentals is an overview on the use of the AMA
Guides, 5th Edition, with a focus on principles of
impairment evaluation, and assessment of spine, upper
extremity, lower extremity, neurological, and pain
impairments.
The seminar is offered in four convenient two hour
sessions, totaling eight hours of superb training. This
seminar is designed for physicians, attorneys, and claims
staff with minimal or no experience in the use of the
Guides. It is also an excellent review for individuals
who may have attended an introductory course and desire
further review and clarification. Most individuals require two
to three learning experiences to grasp the complexities of
impairment evaluation.
At the conclusion of these Guides Webinars you will

assessing musculoskeletal, neurological and pain impairment.

Sixth Edition: A New Hope |

The Sixth Edition, published in December 2007, introduces
new approaches to rating impairment, using a new approach
based on a modification of the conceptual framework of the
International Classification of Functioning, Disability, and
Health (ICF). Although the fundamental principles underlying
the Guides remain unchanged, other changes were also
made to improve internal consistency, promote greater
precision and simplify the rating process.
The approach to impairment evaluation has evolved over the
past fifty years since the Guides started in 1958 with
publication by the American Medical Association (AMA) of the
article, "A Guide to the Evaluation of Permanent Impairment of
the Extremities and Back". The prior Fifth Edition was
published in 2000 and prior to the availability of the Sixth
Edition were utilized in twenty six states including Alaska,
Arizona, California, Delaware, Georgia, Hawaii, Idaho,
Indiana, Iowa, Kentucky, Louisiana, Massachusetts, Montana,
Nevada, New Hampshire, New Mexico, North Dakota, Ohio,
Oklahoma, Pennsylvania, Rhode Island, Tennessee, Utah,
Vermont, Washington and Wyoming.
Many states require the use of the "most recent Edition" of
the Guides either by statute or code; therefore, states
that are expected to implement the Sixth Edition immediately
include Alaska, Hawaii, Kentucky, Louisiana, Mississippi,
Montana, New Hampshire, New Mexico, Ohio, Oklahoma,
Pennsylvania, Rhode Island, Tennessee, Vermont and Wyoming.
The Sixth Edition will be the immediate new standard for
Federal, Longshore and Harbor Workers Act, automobile casualty
and personal injury cases. The most recent edition is also
expected to remain the standard for automobile casualty and
personal injury cases, both domestically and internationally.
Some of the countries that use the Guides include
Australia, Canada, Hong Kong, Korea, New Zealand, and South
Africa.
Some of the recent changes included in the Sixth Edition
include:
- Standardized assessment of Activities of Daily Living
(ADL) limitations associated with physical impairments.
- Functional assessment tools to validate impairment
rating scales.
- Measures of functional loss in the impairment rating.
- Improvement in overall intra-rater and inter-rater
reliability and internal consistency.
- ICF classification uses five impairment classes with
Class 0, normal, to Class 5, very severe.
- Within the ICF model, "diagnosis-based grids" (DBI) were
developed for each organ system with Class 0-5 based
primarily on the diagnosis.
- Final impairment value determined by adjusting the
initial impairment rating given by factors that may include
physical findings, the results of clinical tests, and
functional reports by the patient.
It is hoped that
the Sixth Edition will benefit all stakeholders by minimizing
conflict and improving decision making; however whether this
will occur is not yet known. The process of defining
impairment or the complexities of human function is not
perfect; however, the Sixth Edition should simplify the rating
process, improve accuracy and provide a solid basis for future
editions of the Guides.
