Use this page to log into your account or, if this is your first online referral, create an account:

To create an account,
enter your email address in the right hand box and
click 'Submit'.

You will be re-directed to a screen that informs
you an email has been sent. This validates your email address.

You will immediately receive
an email from
referral@impairment.com
Click on the link at the bottom of the email to access the 'SiteSignup' page and complete your registration.

At the 'SiteSignup'
page, complete all fields and click “Submit”.

After clicking Submit,
you will arrive at your Home page. You can refer a
new case by clicking on the button “Create New
Case”.

Thereafter, case
referrals can be made by entering your user name and
password in the boxes below.
